Our Group Diabetes Education program provides practical, supportive learning for individuals managing diabetes or prediabetes. Led by a qualified healthcare professional, sessions focus on building everyday skills to improve blood sugar control and overall health. Program Includes: Diabetes basics and disease understanding Blood sugar monitoring and goal setting Nutrition guidance and meal planning Medication overview and lifestyle strategies Group discussion and peer support Who It’s For: Individuals with diabetes or prediabetes Newly diagnosed patients Those seeking better diabetes self-management Requirements: No prior diabetes education required Physician referral may be required for insurance billing (if applicable) What Participants Receive: Group diabetes education sessions Educational materials and resources Practical tools for daily diabetes management Learn Together. Diabetes Education Session Policy Purpose The purpose of these classes is to: Clarify information regarding the management of diabetes. Explain how to properly utilize prescribed medications. Empower participants to advocate for themselves following a diabetes diagnosis. Important Notice These sessions do not constitute medical advice and are not a substitute for physician appointments. You are responsible for managing your blood sugar levels with the assistance of a licensed physician or Advanced Practice Provider (APP). In group discussions, anonymity will be required to maintain confidentiality. Refund Policy Sessions cannot be refunded once they have started.
